Painted Rock Family Medicine would like your experience at our clinic to run as smooth and as quick as possible. We have provided you with a list of documents you can print, fill out and sign prior to your clinic visit. If you choose to go over these documents while in clinic, please arrive 10-15 minutes early.

You can email your forms to info@paintedrockfamilymedicine.com or simply just bring them to your scheduled appointments. We have also provided you with our clinic Policies and Procedures.
